Substitutes are paid directly by our district. If you have been APPROVED as a substitute teacher by the ESCCO, please complete a payroll packet BEFORE you accept your first assignment with our district. Please download the following forms, complete and return to Gahanna-Jefferson School District Administrative Office, Human Resources, 160 S. Hamilton Road, Gahanna, 43230

• HR Personal Information Form
• Reasonable Assurance Letter 2010-11
• Pre-Employment Records Check 
• Federal Tax Form
• Ohio Tax Form -- (School Tax Finder)
• School District Residence and Local Income Tax Form -- (Municipal Tax Finder)
• Direct Deposit Form  (a voided check must be attached)
• State Teachers Retirement Form  (leave "first date on payroll" blank)
• State Teachers RETIREE Form  (only print and complete if you have retired from teaching)
• School Employees Retirement Form  (only print and complete if your are NOT a current teacher)
• Social Security Windfall Form 
• Employment Eligibility Form  (You only need to complete and print page 4 and you must provide a copy of your driver's license along with a copy of your social security card, birth certificate, or passport)
• Homeland Security Form -- (view TEL list - you do not need to print this list)
• Staff Network and Acceptable Use and Safety Agreement Form -- (view Agreement - you do not need to print the Agreement - just the form)
