1.) BCI/FBI Background Checks

Fingerprinting for state (BCI) and federal (FBI) background checks can be completed by the Office of Human Resources. The cost is $51, cash or check. Appointments are required. To schedule an appointment, please call (614) 478-5565 or email HullR@GJPS.org. (Please note, if you have current background check results that are within one year old they can possibly be accepted, but they also must have been sent electronically to the Ohio Department of Education at the time of completion.)

2.) CPR

**UPDATE -- VIRTUAL FORMAT FOR CPR CLASSES

Note, if you do not participate in the OhioHealth class, then solely online courses will not be accepted.

Please review the following information from OhioHealth on how to complete and register for CPR training:

OhioHealth Coaches Education Schedule

Frequency:  This training must be done once every two years.

3.) Fundamentals of Coaching

This course is online through the National Federation of State High School Associations (NFHS).  The cost is $65.

Frequency:  This is a one-time only training.

4.) Concussion in Sports

This free course is online through the NFHS or the Centers for Disease Control and Prevention (CDC).

Frequency:  This training must be done once every three years.

5.) Sudden Cardiac Arrest

This free course is online through the Ohio Department of Education (ODE) using the Learning Management System (LMS), which can be accessed through individual OH/ID accounts.  On the LMS homepage at top right, select the Course Catalog.   Search for “sudden cardiac arrest” to find the course.

Per ODE, some OH/ID account users may have difficulty accessing the LMS link.   In that case, please watch the video and read the handout listed under the "for coaches" section on the Ohio Department of Health's website.  After you have watched the video and read the handout, please complete the attached Sudden Cardiac Arrest Acknowledgement Form and submit it as your certificate of completion.

*Please note, the NFHS Simon's Heart course cannot be accepted!

Frequency:  This training must be done once every year.

6.) Pupil Activity Course

In order to apply for a Pupil Activity Permit (PAP), you must first take part in an ODE-approved or nationally-approved pupil activity program (unless you hold a current medical license; please notify the Athletic Office and provide a copy of your active credentials).

OhioHealth provides ‘live’ pupil activity training free for Gahanna-Jefferson coaches.  Visit www.ohiohealth.com/sports-medicine-classes for location, date and registration information.  You may take a live training course elsewhere at your own expense as long as it is an ODE-approved program (visit ODE’s website for more details).   If you prefer to take an online course, a nationally-approved program, First Aid, Health and Safety for Coaches, is available through NFHS.  The cost is $45.

Frequency:  This training must be completed each time you renew your PAP (once every three to five years, depending upon the length of your permit).

7.) Pupil Activity Permit

*Applying for a Pupil Activity Permit should be the final step after all of the other listed requirements have been fulfilled and submitted to the Athletic Office.*

Applications must be completed online through ODE’s website using an OH/ID account.   For the application process, you will need to reference our District’s IRN #046961.   The cost is $45 for a 3-year permit; to certain teachers who hold specific types of educator licenses, 4-year and 5-year permits are available at $60 and $75, respectively.

When you submit and pay for your application through ODE, it will automatically be forwarded to the Office of Human Resources for an electronic signature and verification of requirements.   Once verified, ODE will finalize and process your application, then email a copy of the permit directly to you as well as to the Office of Human Resources.

Frequency:  This permit must be renewed once every three to five years, depending upon if you hold an educator license.  For non-educators, the frequency is once every three years.

8.) Register on FinalForms

Once you are registered as a coach by the Athletic Department, you will receive a confirmation email for FinalForms.  FinalForms is used to hold student-athlete medical information, to track eligibility, and to monitor your certifications/requirements.  All coaches must be on FinalForms.
