Yes. Free/Reduced Applications need to be completed yearly to qualify for free or reduced-price meals. 

No. All children in the household go on one application, even if they attend different schools in the district. All people living in the household should be listed and any income earned must be listed.

Yes! There is a "Consent to Share" form attached to the application that you can complete to receive your instructional, athletic and course fees at a free or reduced price if you qualify for free or reduced-price meals. It should be turned in with the completed application.
